MEMO

In the just concluded year, with the development of committee system in our organization we were able to handle relevant programs geared towards the fulfillment of our mission. We hope to continue this year what we have started To achieve this, the need for funds is obvious. This is why we are taking measures to ensure, we have the funds to sustain our operation. For this purpose we will be collecting from you the following fees.

a. The National Membership Affiliation Fee

The fee will signify your affiliation to the national organization. You have been paying this before at six hundred pesos (P 600.00) every two years. We just made its payment mode annually at P 300.00 per year.
The collected fees will all go to general fund of DSAP used for operational expenses.

b. Publication fee of P100.00

The publication fee will be used for our Monthly Update newsletter. This will enable the national office to keep you a breast with the activities of the association as well as to inform you of the industry's current situation.

c. National Affairs Fund P100.00

The National Affairs Fund will be ear marked for activities related to the legislation, policies and issues affecting our business. These activities include dissemination of information, researches, public relations matters, lobbying and other related activities that will bring about awareness of the issues and the moves towards their resolution.


To summarize, the fees shall be as follows

National Membership Affiliation Fee 300.00
Publication Fee 100.00
National Affairs Fund 100.00
Total dues 500.00


The New ID card

A new membership card will be issued this year to all updated members. It will be an RF card complete with the picture of the member. This will be used for all DSAP functions, validation of membership, inquiring on membership's data, status and bills etc. It will also serve as your national convention ID.
To acquire your ID, simply submit to the National office the following:
a) Fully accomplished membership form with attached documents
b) Proof of payment of the annual dues
c) 2x2 ID picture with white background

To accomplish the above mentioned requirements please take note of the following:
• Membership form can be downloaded through DSAP website – http://www.dsaph.org
or get a copy from your Chapter President
or you can request DSAP Office to fax you one.

• Submission may be personal or through mail or e-mail.
