How To Be a Member

Those who wish to join DSAP shall fill up a membership form (download link below) and submit together with the corresponding fee. The member will be affiliated with the local chapter where his main store/office is located and with DSAP National.

Requirements

For new member:

  • Filled-up Membership form.
  • FDA LTO Certificate Mayor’s Permit (latest)
  • DTI Business Permit Certificate or SEC Certificate for Corporation
  • PRC ID card of registered Pharmacist
  • Colored photo of owner and/or official representative (2×2)
  • Any Government issued ID of Owner
  • Frontage photo of drugstore with signage location of drugstore
Types of Memberships
(1) REGULAR MEMBER
  • Regular Member : With 1 up to 3 branch/s @ PhP1,300.00 
  • Regular Member: with 4 or more branches @ PhP3,000.00
(2) ASSOCIATE MEMBER
  • Wholesalers @ PhP – 3,000.00
  • Franchisors @ PhP – 3,000.00
  • Distributors@ PhP – 3,000.00
